The transfer process works rather smoothly.  Basicly, you submit a request
with your registrar to transfer the domain.  For us, you do that simply by
extending the length of registration an additional year, but other RSPs
may charge different pricing I suppose...

The RSP then sends mail to the contact of that domain (as listed in the
WHOIS record) asking for them to confirm the transfer.  Once that is
done (for us, it's done by visiting a particular URL), the transfer
request is sent to your previous RSP asking for confirmation.  Once they
ack that, the record is updated and is pushed out just like any other
change.

I've done dozens of transfers and they have all happened quite smoothly.
