Hahahahaha!! I had a machine that did that /for/ me for some reason!  I
never /did/ find out where it came from, but it caused me to sort of go
through the horror that you are going through...  So I finally
remembered that I might have a solution for your problem...

Anyway, I found my solution (under slackware) in the
/usr/X11/lib/X11/xinit directory (Actually /var/X11R6/lib/xinit for the
picky ones). The xinitrc file calls xmodmap in two places.  One is in
that same directory, where my .Xmodmap file contains:

  keycode 22 = BackSpace

The other is in the user's home directory, but I don't use that file. 
This fixed the 5/% key problem for me, though I never did find out why
it set it that way to begin with... :(
