Yes I did.  I still haven't ruled it out.  I tried pristine floppies out of 
the box, formatted on my Linux box (using win 98), formatted using my 
Windows development box, formatted using my wife's computer.  I verified 
the disks using Norton Disk Doctor and all but a few had bad sectors.
