I use http://fastmail.fm. A one-time $15 payment gives you
authenticated SMTP and imaps service (as well as sophisticated
filtering/forwarding capability). The only potential downside is
bandwidth/storage limits (I think I'm limited to 16MB storage and
80MB/mo bandwidth), but those can be upped substantially for very little
money (ISTR that $20/year gets you 600MB/300MB, and $40/yr gets you
2GB/1GB). They've been incredibly reliable in the 4 years I've been
using them, and I'd recommend them without hesitation.
